Service Description

As we begin counselling together it is important that you and I both have all the information we need to work together. Important paperwork needs to be completed and discussed prior to engaging in therapeutic processes. It is also very helpful to gather the background information that is meaningful for our work together.

Cancellation Policy

Upon booking you will be required to fill out an In Take Form to be kept on your file for safety and insurance purposes. You will also be emailed an Informed Consent form that includes information about our policies and your rights as a client. These will both need to be completed and submitted to Choice Counselling prior to our Initial session together. My fee is $120/hr due at the beginning of each session. You can pay by etranser to geneva.jacobs@outlook.com or paypal: Geneva Jacobs or via my website or cash if your seeing me in person. If for any reason you need to cancel a session, please notify me by email at least 24hrs prior to our appointment. Otherwise, missed appointments will be charged the scheduled session fee.
